How it works:

1. Fill out the form below with your contact information and that of your loved one. Tell us their story and describe the financial help that could change their situation.

2. If the application is selected, our team will contact you first to better understand the situation.

3. Together, we'll set up the surprise. We'll contact your loved one — sometimes even with you on the line — and capture that unique moment on video.

Throughout the year, these stories will be shared on our social media channels to inspire more acts of solidarity.
